Wellness Haus offers Advanced Lab Testing services, Virtual Care services, and Dr. Hershberg's Medical Weight Loss Program, all designed to support general wellness, health optimization, and various healthcare needs. By participating in these programs, you acknowledge that they are intended for informational purposes only and are not substitutes for professional medical advice, diagnosis, or treatment by a healthcare provider. ADVANCED LAB TESTING
Wellness Haus provides Advanced Lab Testing services intended to support general wellness and health optimization. The Advanced Lab Testing report is for informational purposes only and is not intended to diagnose, treat, cure, or prevent any disease, illness, or medical condition, nor should it be considered a substitute for medical or clinical advice, diagnosis, or treatment by a healthcare provider.By participating in the Advanced Lab Testing program and receiving the Personalized Care Report, you acknowledge and agree that the information contained therein is based on the laboratory tests ordered by the Nurse Practitioner, which were selected based on your health needs and wellness goals discussed during your consultation. The results presented in this report are intended to provide an overview of your health and wellness status and to inform recommendations to help you optimize your overall health.This report should not be relied upon as the basis for any medical or clinical decisions, does not provide diagnosis or treatment recommendations, and should not be used in place of a comprehensive assessment by a qualified healthcare provider. If you have any concerns about your health, we strongly encourage you to consult with your primary healthcare provider or another qualified healthcare professional.Although the Advanced Lab Testing Program is based on established medical guidelines and evidence-based best practices, no screening test can guarantee complete accuracy or predict future health outcomes. Our Virtual Care providers are committed to delivering quality care remotely, right from the comfort of your home. With same-day appointments available, you can receive the care you need when you need it most.
DR. HERSHBERG'S MEDICAL WEIGHT LOSS PROGRAM
The Medical Weight Loss Program, designed by Dr. Hershberg, aims to support your wellness and weight loss goals. This program, optionally coupled with Ozempic, is intended to provide an outline of a health and weight management strategy based on your personal health information and discussed goals. The program does not diagnose, treat, cure, or prevent any disease or medical condition, nor should it be used in lieu of professional medical advice, diagnosis, or treatment.By participating in Dr. Hershberg's Medical Weight Loss Program, you acknowledge that the recommendations are based on the information provided during your consultation. However, this program does not replace the need for regular health checkups with a healthcare provider. The weight management strategies provided in the program are intended to aid you in optimizing your overall health, and should not be used as the sole basis for any medical decisions or actions.The effectiveness of this program may vary between individuals, and no guarantees can be made regarding the results. Therefore, any risks, losses, or damages incurred as a result of adhering to this program are solely your responsibility.

SERVICE #1 - ADVANCED LAB TESTING
By selecting the "I Accept" button, you consent to undergo Advanced Lab Testing as part of the Advanced Lab Testing Program. You confirm that you have read and accept the purpose, conditions and and limitations to the Advanced Lab Testing Program as described above. In addition, you consent to the collection, use, and disclosure of your Personal Health Information (PHI) as necessary to provide the Advanced Lab Testing services. You may withdraw your consent to participate in the Advanced Lab Testing Program or to the collection, use or disclosure of your PHI by contacting Wellness Haus. By selecting the "I Accept" button, you agree that your electronic signature is the legal equivalent of your manual signature on this Agreement. You consent to release, indemnify, and hold harmless the Advanced Lab Testing Program, its staff, affiliates, and service providers, including Nurse Practitioners or other health professionals, from any and all claims, damages, liabilities, or losses that may arise from your use or reliance upon the information provided in the report.
SERVICE #2 - VIRTUAL CARE SERVICES
By selecting the "I Accept" button, you consent to participate in the Virtual Care Services program offered by Wellness Haus. You understand that Virtual Care Services involve the use of electronic communication to enable healthcare providers to remotely diagnose, treat, and manage your healthcare needs. You acknowledge that Virtual Care Services may not be appropriate for all medical conditions and that certain limitations may apply.
SERVICE #3 - DR. HERSHBERG’s MEDICAL WEIGHT LOSS PROGRAM
By selecting the "I Accept" button, you consent to participate in Dr. Hershberg's Medical Weight Loss Program. You confirm that you have read and understood the purpose, conditions, and limitations of the program as described above.You consent to the collection, use, and disclosure of your Personal Health Information (PHI) as necessary to provide the Medical Weight Loss Program services.You may withdraw your consent to participate in the Medical Weight Loss Program or to the collection, use, or disclosure of your PHI by contacting Wellness Haus. You consent to release, indemnify, and hold harmless the Medical Weight Loss Program, its staff, affiliates, and service providers, including healthcare providers, from any and all claims, damages, liabilities, or losses that may arise from your use or reliance upon the information provided in the program.
You consent to the collection, use, and disclosure of your Personal Health Information (PHI) in connection with the provision of Virtual Care Services, [and as set out in our Privacy Policy]. You understand that before proceeding with a consultation, the healthcare provider will need to collect PHI about you in order to determine whether proceed with the virtual appointment. If you proceed, your healthcare provider will need to collect, use or disclose PHI as necessary to You acknowledge that the use of technology may increase the risk of your PHI being unintentionally disclosed or intercepted by third parties. Although Virtual Care Services are provided over a secure platform and reasonable efforts are made to protect the privacy and security of your PHI, you understand that it is not possible to completely secure electronic information.
You understand that the healthcare provider will determine the appropriate course of action based on the information you provide during your virtual appointment. If the provider determines that they cannot help you, your consultation will not start, and you will not be charged. The provider may advise you to visit a family practice, walk-in clinic, urgent care centre or emergency department instead.By participating in the Virtual Care Services program, you acknowledge that the healthcare provider is relying on the accuracy and completeness of the PHI that you provide and has no role or responsibility for determining the accuracy of the information provided by you. You understand that if the health care provider providing Virtual Care Services recommends treatment, prescribes medication or orders tests, you will need to provide your informed consent. You agree that you are responsible for following any medical advice, treatment plans, or follow up recommendations provided by the healthcare provider during your virtual appointment. You acknowledge that it is your responsibility to consult with your primary healthcare provider or another qualified healthcare professional if you have general health concerns, need comprehensive care or if you have questions about the information received during your virtual appointment.By selecting the "I Accept" button, you agree that your electronic signature is the legal equivalent of your manual signature on this Agreement. You consent to release, indemnify, and hold harmless the Virtual Care Services program, its staff, affiliates, and service providers, including healthcare providers, from any and all claims, damages, liabilities, or losses that may arise from your use or reliance upon the information provided during your virtual appointment.
